The Sago Mine officials who miscommunicated that there were surviviors in the mine. Remember that? The media went nuts "They're alive, they're alive, it's a miracle!" Only to have their world shattered because of miscommunication from officials. The media looked very foolish.
"J. Davitt McAteer, a former MSHA director who is chairing the session, issued a statement yesterday afternoon emphasizing that no one person "was responsible for the tragic miscommunication." Instead, he said, it was "a systemic problem, and not the result of individual error or carelessness."
